PARENTAL RELEASE FORM
I/we the parents of the child listed on the registration form hereby give permission for my/our child to participate in the baseball activity as indicated on the registration form.
I/we understand that participation in baseball may result in serious injury, and that protective equipment does not prevent all injuries. By my/our signature(s), I /we do hereby waive, release, absolve, indemnify and agree to hold harmless Steve Balboni Baseball School, LLC, their employees, sponsors, organizations and volunteers for any claim arising out of any injury to my/our child as a result of his/her participation in this activity.
Further, I/we do_____ do not_____ give permission for photos of my child's participation in Steve Balboni Baseball School, LLC activities to be used in connection with publicity articles/photos appearing in Steve Balboni Baseball School, LLC web site, newspapers and or television.
